Depending on the child's age, parents and guardians have different levels of access to their child's MyChart information:

Full Access: 0-12 Years Old

Parents and guardians have full access to all MyChart features through their own MyChart log in. A child age 12-years or younger cannot have a personal MyChart account.

Proxy Access to a Teen's MyChart Information (age 13-17)

Depending on the child's age, parents and guardians have different levels of access to their child's MyChart information:

Limited Access: 13-17 Years Old

At 13, teens may create their own account with full access.When the child turns 13, access is automatically reduced to "limited" for parents and guardians who already had full proxy access. Features such as medications, lab results, appointment scheduling and previously sent messages are disabled or reduced due to state laws, which allow for minors' rights to health information privacy.Limited access for parents and guardians includes the ability to:

  • View some vaccination records
  • Send secure messages to some providers
  • Pay bills

Adult Patient Authorizing Access to Their Own MyChart Account (age 18+)

Adults (age 18 and older) can authorize another person to access their MyChart information to help with their health care needs. Proxy access can be stopped at any time by the proxy or the patient.

Levels of Adult Proxy Access:

  • Full Access:Access to all MyChart features.
  • Scheduling Only:The ability to view and schedule appointments only.
  • Incapacitated Adult:For adults who are not able to make their own medical decisions, proxy access can be provided to the legal representative.

Incapacitated Child Proxy Access

For a child who is not able to make his or her own medical decisions, full access can be provided to the child's legal representative. This is called incapacitated child proxy access.Downloadand print the Incapacitated Child form. This form must be submitted at a Johns Hopkins office along with legal documentation and verification from two medical physicians.

Incapacitated Adult Proxy Access

  1. Downloadand print the Incapacitated Adult Proxy Access form.
  2. Complete and sign the Incapacitated Adult Proxy Access form and submit it to a Johns Hopkins staff member along with documentation supporting your legal representative status (court order or other legal document).
  3. Once the legal documentation has been reviewed and approved, Johns Hopkins staff can complete the proxy set-up process and create your MyChart proxy account.
  4. Johns Hopkins can provide you with the access code necessary to set-up your MyChart account.
